Formation of Medicare Shared Savings Program, Affiliated Physician Organization, and Affiliated Medical Foundation

Advised Hartford HealthCare on the (i) formation of a Medicare Shared Savings Program accountable care organization, including drafting the governance documents, provider participation agreements and assisting in the preparation of the successful CMS application and program operational documents; (ii) the formation of its affiliated physician organization comprised of private physicians who commit to clinical integration and joint patient management and shared risk; (iii) the formation and operation of a health care system affiliated medical foundation comprised of primary care and specialist physicians, including regularly advising on regulatory, reimbursement and organizational issues.
